Certificate/Medical Certificate Issuance

For Outpatients

  1. Request for a medicate certificate at the medical department concerned
  2. The medical certificate is filled out by the doctor
  3. Medical certificate issuance

    (eception desks #2 and #3 in the main building; Admission/Discharge Desk in the Children Building; or Admission/Discharge Desk at the Rehabilitation/Arthritis center)

For Inpatients

  1. Request for a medical certificate at the ward
  2. The medical certificate is filled out by the doctor
  3. Medical certificate issuance

    (reception desks #2 and #3 in the main building; Admission/Discharge Desk in the Children Building; or Admission/Discharge Desk at the Rehabilitation/Rehabilitation Center)

How to Request for a Medical Certificate

  • Only the patient him/herself may request for the issuance of a medical certificate, in accordance with the Medical Service Act (it will not be issued to any other individuals). However, a medical certificate may be issued following the submission of the required documents if the patient is in death or a coma.
  • The required documents for reissuance are identical to the documents necessary for the initial issuance.

Required Documents for Issuance of Medical Certificate

  • Requested by the patient: The patient’s ID card(resident registration card(alien registration card), passport, driver’s license or any other ID cards issued by a government agency)
  • Requested by the patient’s guardian: The patient’s ID card, the guardian’s ID card and family relations certificate
  • If the patient is a minor: The guardian’s ID card and family relations certificate
  • When an agent (e.g. insurance company, non-family member) is making the request: A copy of the patient’s ID card (except in cases where the patient is under the age of 17), the agent’s ID card, a signed written consent from the patient and healthcare proxy (filled out/signed by the legal representative if the patient is under the age of 14; a document confirming the legal representative’s status such as a family relations certificate is required)

Matters Concerning the Issuance of Medical Certificates

  • Issuance of certificates: Main building (reception desks #2 and #3) / The Children building (Admission/Discharge Desk) / Rehabilitation/Arthritis Center (Admission/Discharge Desk)
  • Issuance of a medical certificate (health diagnosis certificate): Check in at the General Health Examination Center (T.280-8530) and receive the necessary tests and examination. A medical certificate will be issued after 3 p.m. on the next day or the day after
  • There is a fee for issuance of a certificate (the fee varies depending on the certificate type).
